Grant Forms for Individual Artist Grants
In addition to completing the application in eGRANT, individuals interested in applying for the Teaching Artist Roster must provide reference forms completed by four different teachers or administrators who are familiar with the applicant’s work as a teaching artist. The reference must submit the form separately to MAC before the March deadline through an email to Charlotte Smelser, Arts Education director: csmelser@arts.ms.gov.
Grant Final Report Forms For Organizational Grantees
All grantees must complete a final report. Unless you have applied for a Special Initiative grant, go to your dashboard in eGRANT to complete all final reporting.
Additional Forms
MAC Grant Change Form – Changes in your approved project activity must be approved before implementation. Submit this Grant Change Form if any of the following apply to you:
- If your organization has a change of personnel associated with your grant request. (Only required if the new personnel is listed as either the Authorizing Official or the Project Director.)
- If you need to update the user account information in your eGRANT account. Do NOT make unauthorized changes to user account information in eGRANT.
- If you need to make a change in your approved project activity at any point during the grant period such as change of date, a change in the consultant/artist the application proposed to engage, etc.
- If out of an abundance of caution, you have chosen to alter, postpone, or cancel your planned grant activities in response to the COVID-19 outbreak.
Send the completed form to your MAC Program Director for review & approval.
MAC Logos and Sample Press Releases
MAC grantees are required to acknowledge the agency’s support in all of the published material or publicity related to a funded project.
Sending out a press release about your grant helps you get the word out about your accomplishment. Below are links to press release templates that you can use to announce your grant to the media.
